What to expect at your visit?
Our goal at Dr Daisy Chemaly & Associates, is to provide you with excellent care in a comfortable and relaxed setting. Your general dentist has trusted us with your care and we are committed to delivering and exceeding your expectations.
Pre-visit requirement:
- A referral letter and current panoramic x-ray (within 1 year) from your referring dentist is required and must be brought with you. This will assist our doctors in evaluating and preparing for your visit at our practice.
- Personal and medical information is also required as well as a list of all current medications that you are taking.
- Insurance information for electronic submission.
- We offer free parking at our facility
- Your first visit will be a consultation (unless it is a emergency ) with one of our Specialist. Initial visit at our practice:
- If you are under the legal age of 18 – you must be accompanied by your parent or legal guardian (s).
- Consultation visit will involve a thorough diagnosis, explanation of recommended treatment and options, including risks and benefits of treatment
- One of our patient care administrators will further explain and review what is involved with your treatment, all associated cost, preparation of a estimate and will schedule your appointment for surgery. You will have an opportunity to address any questions or concerns at this visit.
- On some occasions, your surgery may take place the same day ( depending on the severity) Surgical appointment:
- Follow all pre surgical/anesthesia instructions
- A responsible adult must accompany you and drive you home after your surgery
